Embolic Phenomena During Revision Total Hip Arthroplasty: Diagnosis with Transesophageal Echocardiography
نویسندگان
چکیده
Embolic material was seen in the right atrium during one or more phases of revision total hip arthroplasty (THA)in 17 of 21 ASA Class II or Class III patients (77%) who were monitored with transesophageal echocardiography (TEE). The largest volume of embolic material occurred during preparation and insertion of cemented femoral components. Multiple occurrences of embolization during the procedure appear to have an additive effect which precipitated an increased use of the intensive care unit and the need for greater pulmonary support postoperatively, p < 0.05. Although fat and marrow elements are suspect, the exact nature of the embolic material that has been observed is presently undefined.

Embolic events during total hip arthroplasty: an echocardiographic study.
We performed transesophageal echocardiography on 88 patients treated with cementless or hybrid total hip arthroplasty (THA). We observed a significantly higher frequency of severe embolic events among patients without cup holes during insertion of press-fit cups (46.0%) than in patients with holes (15.6%) (P<.005). During stem insertion, severe embolic events occurred in 61.5% of hybrid operati...
متن کاملEffectiveness of the ROBODOC system in preventing intraoperative pulmonary embolism.
Intraoperative pulmonary embolism occurs not only during cemented but also during cementless total hip arthroplasty (THA). We determined whether the ROBODOC femoral milling system can reduce intraoperative pulmonary embolism, by using of transesophageal echocardiography and hemodynamic monitoring. We did a prospective clinical trial with 71 patients (75 hips) who were divided into 2 groups: gro...
متن کاملAbnormal echogenic findings detected by transesophageal echocardiography and cardiorespiratory impairment during total knee arthroplasty with tourniquet.
BACKGROUND In patients undergoing total knee arthroplasty, intraoperative pulmonary embolic events are rare, and most occur following tourniquet deflation. This embolization can be observed using transesophageal echocardiography. However, the authors have encountered sudden decreases in arterial oxygen partial pressure while a tourniquet is still inflated. Therefore, the current investigation w...
متن کاملFatal Cobalt Cardiomyopathy Following Revision Total Hip Arthroplasty – A Brief Report with Review of Literature
Ceramic bearing surfaces are being increasingly used in young patients undergoing total hip arthroplasty. However, failures have been reported including fractures even with the newer third generation ceramics. The recommendedtreatment for fracture of ceramic bearing surfaces is complete synovectomy and revision total hip arthroplasty. However, disappointing results have also bee...
متن کاملTotal Hip Arthroplasty with Modular Stem for Dysplastic Hips in South Asian Population
Introduction: Optimum component positioning and orientation is required to optimize the functional result during total hip arthroplasty for dysplastic hips. Patients and methods: Sixty-two patients (66 hips) including 33 males and 29 females underwent total hip arthroplasty using modular stem prosthesis at an average age of 40.6 years (range 17 to 49 years). Nineteen hips were clas...
